Medical and Social Services involves assisting the client in acquiring the needed information for long-term care options. These services are provided by social workers or case managers:
- Suggesting community programs
- Giving education about how to get services to patients and caregivers
- Assisting patients in finding funds to help with costs of medication, food, housing and
transportation - Conflict resolution between the caregiver, family and patient
- Providing counseling for the patient and the caregiver in dealing with illness
